Transitioning children with complex medical needs from pediatric to adult care is often an informal, complicated process that places stress and burden on the patient, families, and clinicians. This roundtable seeks to highlight the systemic barriers to the process and generate a pathway to solutions designed to breakdown these barriers.
Facilitated by a partnership between Gillette Children’s Specialty Healthcare, The Chloe Barnes Advisory Council on Rare Diseases, the Minnesota Chapter of the American Academy of Pediatrics, and the Minnesota Department of Health.

Roundtable Panelist Biographies (more to come) Kurt Elting-Ballard, MD, MPH | Regional Medical Director | Minneapolis Region and Abbott Northwestern MarketOriginally from California, I completed my residency training at the University of Minnesota in Internal Medicine and Pediatrics. I have worked for Allina Health for 22 years and still practice part time at Allina Health Shoreview Clinic. Currently I serve as a Regional Medical Director for our Minneapolis region and Abbott Northwestern Market. As part of this role, I am the Medical Director for Pediatrics for the Allina Health Group and chair our Ambulatory Pediatric & Adolescent Council.
Gretchen Karstens, MD | Pediatrician | St. Luke’s Pediatric AssociatesMy name is Gretchen Karstens and I’m a general pediatrician living and working in Duluth at St Luke’s Pediatric Associates. I grew up in rural Northern MN and am thrilled to be home for the past 16 years enjoying the beautiful North Shore of Lake Superior. I received an undergraduate degree in psychology from Smith College in Northampton, MA and went to medical school at the Medical College of Wisconsin in Milwaukee. I completed my pediatric residency at Children’s Hospital Wisconsin in Milwaukee. I work with my patients and families on collaborative decision making through patient-and-family centered care. The best part of my job is watching my patients grow up while enjoying the privilege of being part of their care team. Transitioning “my kids” to “adult doctors” is hard to do, I miss them.
Tom Kelly | VP Marketing and Innovation | UnitedHealth GroupAs the father of a child with a rare genetic disorder, Tom joined UnitedHealth Group in 2009 with the hope of improving health care experiences for families like his. He ultimately helped found the UnitedHealth Group Special Needs Initiative (SNI) – a team of nearly 500 people dedicated to helping families who have children with special health care needs/rare diseases. Now in its 5th year, SNI has improved the experiences for over 150,000 families while lowering their total cost of care. Prior to UHG, Tom held executive creative leadership positions at prominent national advertising agencies.
I am a mother of three children, my youngest with a very rare, progressive, genetic illness that has left her totally dependent and in a medically fragile state. I worked for 45 years for General Electric in various roles: manufacturing, finance, sales, sales training, and leadership development. After retirement, I joined the Pacer Center team where I had previously volunteered as the Health Information Center Coordinator. I am currently working with MGN, MDH, MCH, and the UOM to create a transition plan for all children with disabilities who are aging out of the pediatric care system. Transition of care is a passion for me as my child, with a pediatric disease onset, is struggling to receive safe, critical care as needed. She is an adult, only by age, and because of this, we have struggled to get attention to her specific health care needs and the fact that this rare disease presents differently in adult onset.

Identify systemic barriers to transition in their own practice.
Develop language to have conversations with their health system to put in place a process to support transition to adulthood for patients with complexity or rare diseases.
